This is a fully remote position, open to applicants in United States.
• Supervise and manage the Care Coordinator SNP team.
• Facilitate health care management and coordination of care for SNP members with intricate and chronic care requirements.
• Ensure the timely completion of Health Risk Assessments, Individualized Care Plans, and Interdisciplinary Care Team delivery.
• Provide direction to staff and handle complex coordination requests from members, providers, or staff.
• Assist in the development and management of SNP Care Coordination programs, projects, workflows, SOPs, and reports.
• Collaborate with supervisors and managers to enhance processes and workflow.
• Act as a resource and liaison for members, providers, community partners, management, and the care coordination team.
• Review program performance reports, identify gaps, and ensure compliance with internal goals and regulatory standards.
• Engage in SNP quality audits, including CMS and vendor audits, report findings, and recommend process improvements.
• Delegate time-sensitive tasks to Care Coordinators.
• Mentor, train, audit, coach, and manage non-clinical Care Coordinator personnel.
• Oversee administrative tasks including maintaining member records, correspondence, case openings, and task creation in the Care Management platform.
• Assist with Case Management team coverage plans.
• Uphold confidentiality of information among healthcare professionals.
• Recruit, select, orient, train, allocate workload, monitor, evaluate, coach, counsel, and discipline assigned personnel.
• Carry out other duties as assigned.
• A minimum of 3 years of experience in Care Coordination.
• At least 1 year of supervisory or lead experience in a healthcare environment.
• High school diploma or GED, along with one to three months of related experience and/or training, or an equivalent combination of education and experience.
• Capability to analyze data for trends.
• Familiarity with Managed Care Plans and medical terminology.
• Proficient in computer skills, including Microsoft Outlook, Excel, and Word.
• Typing speed of 25 WPM.
• Strong oral, written, and telephone communication skills.
• Ability to apply common-sense reasoning to detailed written or oral instructions.
• Capacity to address problems involving a few concrete variables in standardized situations.
• Excellent organizational skills.
• Ability to reason and execute instructions effectively.
• Strong interpersonal skills.
• Proficiency in spoken and written English.
• Capability to file systematically.
• Ability to perform essential physical functions, including standing, walking, sitting, using hands, reaching, talking, and hearing.
• Ability to frequently lift and/or move up to 10 pounds.
• No licensure required.
