RN Case Manager – Care Transitions, Bilingual Spanish, California RN License Required

This is a fully remote position, open to applicants in California.

📋 Description

• Develop personalized discharge plans in collaboration with patients, their families, and the care team.

• Facilitate care coordination among providers, rehabilitation facilities, and home health agencies.

• Track patient progress and modify plans as necessary.

• Inform and empower patients and their families regarding self-care and follow-up procedures.

• Advocate for patient needs and link them to community support services.

• Engage in care conferences, quality improvement efforts, and interdisciplinary teamwork.

• Assist members in transitioning safely from hospital or skilled nursing facility stays back to their homes.

• Deliver education, support, and coordination that positively influence health outcomes.

• Oversee discharge planning, referrals, and community resource connections.

⛳️ Requirements

• Minimum of 5 years of clinical case management experience, or a comparable combination of education and experience.

• Current, unrestricted RN license in California.

• Willingness to obtain additional state licenses.

• Familiarity with Medicare Managed Care Plans.

• Understanding of insurance regulations.

• Awareness of community resources.

• Proficiency in bilingual Spanish.

• Excellent communication, problem-solving, and organizational abilities.

• Capacity to perform essential physical functions, including frequent standing, walking, sitting, using hands, reaching, talking, and hearing.

• Ability to lift and/or move up to 10 pounds regularly.
