Bilingual Care Coordinator – Spanish, English

This is a fully remote position, open to applicants in Colombia.

📋 Description

• Facilitate communication among patients, healthcare providers, specialists, multidisciplinary care teams, families, and administrative personnel.

• Develop, update, and oversee personalized patient care plans while tracking progress towards clinical objectives.

• Organize follow-up appointments.

• Keep precise electronic health records (EHR/EMR).

• Manage referrals and verify insurance eligibility.

• Respond to patient inquiries through phone, email, or chat.

• Link patients to community resources, home care services, medical equipment suppliers, and financial assistance programs.

• Conduct patient check-ins, monitor adherence to treatment, tackle care obstacles, and educate patients on their prescribed care plans.

• Ensure adherence to HIPAA regulations, patient privacy laws, and standard operating procedures (SOPs).

• Detect treatment delays, care gaps, and recurring operational challenges and escalate them to clinical leadership.

⛳️ Requirements

• A minimum of 3 years of experience in healthcare coordination, medical administrative support, patient navigation, or case management.

• Proficient in Electronic Health Record (EHR) systems.

• Skilled in using HIPAA-compliant communication tools.

• Competent in Microsoft Office Suite and Google Workspace.

• Exceptional verbal and written communication abilities.

• Strong empathetic listening skills.

• Excellent time-management capabilities.

• High level of attention to detail.

• Proven track record of managing multiple care tracks simultaneously.

• Prior experience in remote healthcare or telehealth is preferred.

• Knowledge of medical billing, coding, or insurance verification is advantageous.

• Bilingual skills as required by the patient demographic.

• Ability to comply with HIPAA regulations, patient privacy laws, and SOPs.

• Strong problem-solving and conflict-resolution skills.

• Proactive in follow-through and accountability.

• Adaptable to fast-paced healthcare environments.
