Behavioral Health Case Manager I

This is a fully remote position, open to applicants in Connecticut, +2 more states.

📋 Description

• Conduct telephonic case management within the limits of licensure for members requiring support with behavioral health and substance use disorders.

• Apply appropriate screening criteria knowledge and clinical judgment to evaluate member needs.

• Carry out assessments to ascertain individual requirements.

• Formulate care plans that address the objectives and goals identified during assessments.

• Oversee and assess the effectiveness of care plans, modifying them as necessary.

• Facilitate member access to suitable, quality, and cost-effective healthcare.

• Coordinate with both internal and external resources to fulfill the identified needs of members.

• Collaborate with healthcare providers.

• Complete mandatory in-person training sessions while working virtually full-time.

⛳️ Requirements

• MA/MS in social work, counseling, or a related behavioral health discipline, or a nursing degree, alongside a minimum of 3 years of clinical experience in social work counseling, with extensive experience in complex psychiatric and substance use disorder treatment; or any combination of education and experience that would provide an equivalent background.

• A current active unrestricted license such as RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW, or Clinical Psychologist, as required by state law and scope of practice.

• Licensed to operate as a health professional within the bounds of licensure in the applicable states or territories of the United States.

• Preferred experience in case management and telephonic and/or in-person coaching with members facing complex psychiatric, substance use, and/or medical disorders.

• For Government business only, LAPC or LAMFT licensure may be acceptable as permitted by relevant state laws.

• Must possess a license in the state of New York.

• Must be capable of working virtually with members located in Western New York.
