Behavioral Health Care Manager – Licensed Master Social Worker

This is a fully remote position, open to applicants in Texas.

📋 Description

• Act as the primary point of contact for patients participating in the BHI or CoCM program.

• Perform regular behavioral health screenings, such as PHQ-9 and GAD-7, and monitor patient progress over time.

• Offer patient outreach, psychoeducation, and support in between clinical appointments.

• Work in collaboration with the psychiatric consultant and primary care provider to assess patient progress and modify care plans.

• Record patient interactions, screenings, updates to care plans, and psychiatric recommendations in the medical records.

• Oversee adherence to behavioral health treatment plans and facilitate follow-up care coordination.

• Ensure that patient consent for collaborative care is secured and documented prior to billing.

• Help patients navigate barriers to care, including scheduling, transportation, and understanding treatment options.

• Coordinate referrals to specialized mental health providers as necessary.

• Support billing operations by ensuring compliance with time tracking, service documentation, and updates to care plans as mandated by CMS and commercial insurers.

⛳️ Requirements

• Bachelor's or Master's degree in Social Work, Psychology, Counseling, Nursing, Public Health, or a related field.

• Previous experience in behavioral health, care coordination, or chronic disease management is preferred.

• Familiarity with behavioral health screening instruments (PHQ-9, GAD-7, etc.).

• Knowledge of the Collaborative Care Model and/or BHI billing requirements is highly advantageous.

• Excellent communication and interpersonal skills to interact effectively with patients, families, and multidisciplinary teams.

• Skilled in electronic health record (EHR) documentation and case management processes.

• Bilingual skills are considered a plus.

• Must be a resident of Texas.
