Virtual Family Nurse Practitioner

This is a fully remote position, open to applicants in Illinois, +3 more states.

📋 Description

• Provide patient care through telehealth services, including phone triage, video consultations, and email follow-ups.

• Engage in weekly Clinical Rounds and other ongoing learning opportunities.

• Collaborate with both virtual and in-office team members during daily huddles.

• Utilize clinical expertise to deliver outstanding virtual patient care.

• Offer round-the-clock virtual clinical services throughout the year, in coordination with patients' primary care providers.

⛳️ Requirements

• Successfully completed an accredited FNP program and obtained national certification.

• Within the last five years, have worked as an Advanced Practitioner for a minimum of two years in an outpatient primary care environment serving patients of all ages (0+), or two years in urgent care or emergency medicine catering to patients of all ages (0+).

• Hold a current license in IL, NY, NJ, or MA, with the capability to secure additional state licenses.

• For Illinois license: must possess independent practice authority in Illinois, APRN-FPA.

• For New Jersey license: must have Advanced Practice Nurse (APN) status with Full Practice Authority (> 5000 hours in primary care).

• Willingness to work weekday and weekend shifts, including every other Saturday and/or Sunday.

• Flexibility to work current shifts ranging from 8am–7pm EST, 11am–10pm, 12pm–11pm EST, or 1pm–12am EST.

• Exceptional clinical and communication skills.

• Demonstrated clinical proficiency in evidence-based primary care.

• Competence in using technology to provide high-quality care.

• Ability to communicate effectively and deliver care to individuals from diverse backgrounds.

• Strong passion for human-centered primary care.

• Capability to navigate challenging situations with patients and colleagues.

• Preparedness to adjust behavior according to patient needs.

• Must be authorized to work in the U.S.
