• Lead and implement strategic initiatives across the enterprise aimed at enhancing clinical quality outcomes, patient satisfaction (CAHPS), and employee engagement and retention.
• Ensure that all documentation is precise, comprehensive, and defensible.
• Oversee the Clinical Resource/Review Team responsible for validating Plans of Care, ensuring OASIS accuracy, and maintaining coding integrity.
• Supervise the Quality Outcome Specialist team tasked with managing quality and outcome initiatives.
• Drive performance in HHVBP, improve Star Ratings, and work towards reducing hospitalizations.
• Utilize analytics, optimize EMR, and employ predictive tools to enhance outcomes and minimize variability.
• Foster clinician engagement, retention, and productivity while mitigating burnout and improving workflow efficiency.
• Ensure adherence to CMS and accreditation standards.
• Participate in the preparation of quality data reports for the executive team and board of directors.
• A minimum of 5 years of leadership experience in home health or hospice settings.
• Proficiency in OASIS-E, HHVBP, PDGM, documentation integrity, and strategic quality improvement methodologies.
• An active RN license is preferred.
• A Bachelor’s degree or higher is preferred.
