Travel Nurse Practitioner / Physician Assistant

This is a fully remote position, open to applicants in Louisiana.

📋 Description

• Travel within the designated territory to perform in-home evaluations, comprehensive annual wellness examinations, targeted screenings, and other visits, both in patients' homes and virtually.

• Be prepared to travel out of state for periods of up to 2 weeks as necessary.

• Provide guidance and education to patients and their families regarding benefits and programs that promote healthier lifestyles.

• Record primary complaints, diagnoses, medical history, system reviews, examinations, medications, allergies, assessments, and treatment plans.

• Collaborate with primary care providers, specialists, and ancillary services to coordinate patient care.

• Ensure timely completion of documentation and paperwork.

• Adhere to practice-defined quality-of-care standards.

• Offer evidence-based, timely care aimed at minimizing avoidable hospitalizations and enhancing quality of life.

• Prescribe medications, order diagnostic tests, and work in partnership with the patient's Monogram physician.

• Enhance patient quality outcomes, which are measured and reported on a daily basis.

• Close care gaps through preventive monitoring education and coordination of diagnostic testing.

• Support patients in enrolling to access educational videos.

• Participate in integrated care team meetings.

• Evaluate patients' health status.

• Develop and execute treatment plans.

• Conduct follow-ups and assess patients' progress.

• Undertake additional responsibilities as assigned.

⛳️ Requirements

• Active and unrestricted licensure as a Registered Nurse and Nurse Practitioner.

• Board certification in accordance with relevant licensure (NP: ANCC/AANP).

• Current and unrestricted DEA certificate.

• Willingness to travel up to 75% of the time.

• Capability to work independently without direct supervision and practice autonomously.

• Access to transportation, including a valid driver's license and car insurance.

• Proficient in using necessary medical instruments and equipment.

• Familiarity with computer-based data management programs, information systems, medical records, and point-of-interview technology.

• Strong verbal and written communication skills with retail and medical partners, patients, family members, physicians, and community representatives.

• Understanding of federal and state regulations, including HIPAA and OSHA.

• A minimum of 2 years of direct patient care experience.

• Experience in Managed Care/IPA/Health Plan settings.

• Background in conducting wellness visits, focused assessments, or similar comprehensive evaluations, either virtually or in the home.

• Bilingual in Spanish is a plus, but not mandatory.
