Transformation Consultant

This is a fully remote position, open to applicants in California.

📋 Description

• Serve as a coach for the implementation of technology

• Equip health center clinicians and leaders with effective strategies and tools

• Enhance clinical practices by utilizing EHR functionality and data

• Redesign both clinical and operational workflows

• Partner with clinicians and leadership to evaluate existing workflows

• Assist in the rollout of digital health tools

• Oversee multiple projects focused on clinical improvement concurrently

⛳️ Requirements

• Bachelor's degree in healthcare or a related discipline; a master's degree is highly preferred

• At least three (3) years of experience supporting healthcare teams in a similar or relevant position; four to five (4–5) years is strongly preferred

• Outstanding written and verbal communication abilities

• Knowledge of improvement methodologies (e.g., Model for Improvement)

• Experience with clinical quality metrics (e.g., UDS, HEDIS, NCQA, NQF) is preferred

• Understanding of change management and human-centered design principles

• Previous experience with electronic health records and workflow visualization tools; familiarity with Epic is preferred

• Strong facilitation capabilities

• Willingness to travel to urban and rural areas up to 25-50%
