This is a fully remote position, open to applicants in Pennsylvania.
• Perform thorough telephonic and virtual clinical evaluations, which include the analysis and interpretation of biometric, RPM, and IVR data along with trend assessment.
• Create, execute, and modify personalized nursing care plans that align with patient objectives, evidence-based practices, and organizational standards.
• Utilize Motivational Interviewing techniques to engage patients effectively, promoting behavior modifications, adherence, and self-management.
• Recognize clinical risks, changes in patient condition, and urgent scenarios; escalate issues as necessary in line with clinical protocols.
• Collaborate with interdisciplinary care teams (both internal and external) to ensure coordinated care, referrals, and transitions throughout the care continuum.
• Implement proactive measures aimed at minimizing hospital admissions, emergency room visits, and overall care costs.
• Accurately, promptly, and thoroughly document all patient interactions in the electronic health record (EHR).
• Take part in quality improvement projects, chart audits, and performance evaluations to enhance clinical results and care delivery.
• Conduct care management activities in accordance with client-specific workflows, performance expectations, and service-level agreements.
• Ensure adherence to all clinical, regulatory, HIPAA, and organizational standards.
• Nursing degree from an accredited institution (ADN or BSN required; BSN preferred).
• At least 5 years of extensive clinical nursing experience.
• A minimum of 3 years of experience in one or more of the following areas: Care management or care coordination, Home health or transitions of care, Telehealth or virtual care, Chronic disease management (e.g., cardiac, pulmonary, endocrine).
• Proficient in computer skills, including EHR documentation, navigating various systems, and resolving basic technical problems.
• Capability to work within designated time zones and schedules based on client requirements.
• Valid, unrestricted RN license Compact (NLC) license and/or California RN license required, depending on the role.
