This is a fully remote position, open to applicants in North Carolina.
• Employ person-centered planning techniques to collect information, enabling a deeper understanding of the members supported and effectively addressing their individual needs.
• Educate and assist members in recognizing and exercising their rights and responsibilities.
• Diligently carry out Discovery activities (information collection and assessment) as part of the planning process.
• Ensure that members and legally responsible individuals are made aware of available services (including all types of Medicaid and state-funded services), service options (e.g., Individual/Family Direction for Innovations participants), processes (e.g., specific service requirements), and natural and community resources.
• Support members and legally responsible individuals in selecting service providers, maintaining objectivity throughout the process.
• Must possess QP status.
• Fully licensed by the North Carolina governing board that oversees Human Services professions; OR be a fully licensed RN authorized to practice in North Carolina by the NC Board of Nursing with four (4) years of full-time experience with the MH/DD/SA population.
• OR hold a Master’s degree in a human service field from an accredited college or university and possess one year of full-time, pre or post-graduate degree experience with the MH/DD/SA population, or be a substance abuse professional with one year of full-time supervised experience in alcoholism and drug abuse counseling.
• OR hold a bachelor's degree in a human service field from an accredited college or university and have two years of full-time, pre or post-bachelor's degree experience with the MH/DD/SA population, or be a substance abuse professional with two years of full-time supervised experience in alcoholism and drug abuse counseling.
• OR hold a bachelor's degree in a field outside of human services from an accredited college or university and have four years of full-time experience, either pre or post-bachelor's degree, with the MH/DD/SA population, or be a substance abuse professional with four years of full-time supervised experience in alcoholism and drug abuse counseling.
• Must possess a valid driver’s license.
• Must reside in North Carolina and within 75 miles of the Mid-State Trillium counties: Guilford.
• Must be able to travel within the designated catchment area as required.
• Typical working hours: 8:30 am – 5:00 pm; flexible work schedules available for certain roles with management approval.
