Telephonic Nurse Care Associate

This is a fully remote position, open to applicants in Maine.

📋 Description

• Perform telephonic clinical outreach to patients participating in Remote Patient Monitoring (RPM) and chronic care programs.

• Analyze biometric alerts and detect out-of-range readings; escalate clinical issues following established protocols.

• Offer patient education and coaching to enhance adherence to care plans and lifestyle recommendations.

• Thoroughly document patient interactions in EMR/RPM systems, adhering to clinical and regulatory standards.

• Collaborate with clinical management, RN Care Managers, providers, and interdisciplinary care teams to ensure coordinated patient care.

• Assist in patient engagement initiatives aimed at improving monitoring compliance and clinical outcomes.

⛳️ Requirements

• Current LPN/LVN license in good standing.

• Over 5 years of clinical experience, with at least 3 years in care coordination, telehealth, home care, or chronic disease management.

• Preferred experience in supporting patients with conditions such as CHF, COPD, diabetes, or hypertension.

• Excellent telephonic communication, patient education, and motivational interviewing skills.

• Familiarity with EMR/EHR systems and comfort in a technology-driven care environment.

• Capability to work independently within a structured remote clinical workflow.
