This is a fully remote position, open to applicants in Wisconsin.
• Act as the main point of contact for patients who are newly referred.
• Perform outbound outreach to newly enrolled prospective patients to explain the Remote Patient Monitoring programs, services, and benefits offered by AMC Health.
• Connect with patients over the phone to build rapport, address inquiries, and promote participation and adherence to monitoring programs.
• Conduct essential telephonic interventions, such as Welcome Calls and Post-Physician Visit Updates.
• Accurately and thoroughly document patient information within the electronic health record (EHR).
• Work in collaboration with nurse care managers and the wider care team to achieve patient care objectives.
• Monitor hospitalizations and inform nursing staff when patients are discharged as necessary.
• Organize patient assistance services as instructed.
• Address inbound inquiries from patients, clients, and providers.
• Initiate outbound communications with patients, clients, physician offices, and care providers.
• Verify and maintain precise provider contact information.
• Promptly escalate unresolved patient issues or care-coordination challenges to leadership.
• Achieve or exceed established performance metrics and service level agreements (SLAs).
• Preferred: Completion of a Medical Assisting program accredited by CAAHEP or ABHES.
• Minimum experience: At least six (6) months of experience performing general duties as a Medical Assistant OR three (3) years of experience in a clinic or similar healthcare environment with direct patient contact.
• Familiarity with medical terminology and prior experience using electronic health record (EHR) systems.
• Strong technical skills, including the capability to assist patients remotely with health devices and troubleshoot basic issues.
• Must be able to type a minimum of 50-60 words per minute while conversing with patients on the phone.
• Must be available to work schedules that align with client-specific time zone requirements, based on business needs.
• Active CNA certification or equivalent certification (e.g., Pharmacy Technician), in good standing.
• Ability to work efficiently in a fully remote setting.
• Requirement to maintain a dedicated home office that is private, quiet, secure, and free from distractions.
• Reliable internet access that meets the company's minimum technical specifications.
