Telephonic Health Services Coordinator

This is a fully remote position, open to applicants in United States.

📋 Description

• Act as the main contact for patients who have been newly referred.

• Perform outreach to prospective patients who have recently enrolled.

• Connect with patients over the phone to build rapport, answer inquiries, and promote participation in monitoring programs.

• Accurately and thoroughly document patient information in the electronic health record (EHR).

• Work in conjunction with nurse care managers and the larger care team to assist in achieving patient care objectives.

• Monitor hospitalizations and inform nursing staff upon patient discharge when necessary.

• Address incoming inquiries from patients, clients, and providers, and assess service requirements effectively.

• Start outbound communication (via phone, fax, email) with patients, clients, physician offices, and care providers.

• Adhere to all company policies, procedures, HIPAA regulations, and relevant federal and state laws.

⛳️ Requirements

• Completion of a Medical Assisting program accredited by CAAHEP or ABHES is preferred.

• A minimum of six (6) months of experience in general Medical Assistant duties OR three (3) years of experience in a clinic or similar healthcare environment with direct patient interaction.

• Familiarity with medical terminology and prior experience with electronic health record (EHR) systems.

• Strong technical skills, particularly in assisting patients with health devices remotely and troubleshooting basic issues.

• Capacity to type at least 50-60 words per minute while conversing with patients on the phone.

• Must be able to work schedules that align with client-specific time zone requirements, as needed by the business.

• Current CNA certification or equivalent certification (e.g., Pharmacy Technician) in good standing.
