This is a fully remote position, open to applicants in California, +12 more states.
• Conduct synchronous nutrition counseling sessions with patients (60 minutes for the initial session and 30 minutes for follow-ups) to support their individualized medication-assisted weight loss journeys.
• Proactively manage common side effects associated with GLP-1 and AOMs (such as nausea, constipation, reflux, reduced appetite, and risk of lean-mass loss) by implementing personalized nutrition strategies that preserve muscle mass, ensure adequate protein and fiber intake, and facilitate sustainable weight loss.
• Utilize evidence-based behavior change techniques (including motivational interviewing, SMART goal-setting, and CBT-informed coaching) to enhance self-efficacy and promote long-term lifestyle changes.
• Modify care plans in response to medication or dosing changes, insurance coverage adjustments, weight-loss plateaus, or evolving patient goals, in collaboration with the prescribing clinician.
• Follow up with patients through asynchronous messaging to address their questions.
• Coordinate care with Found clinicians and the member support team by leveraging Found’s internal EMR provider messaging function.
• Deliver care to patients with a high standard of personal and professional integrity and ethics, maintaining a commitment to confidentiality.
• Complete session documentation (including ADIME notes, patient communications, and care team follow-up messages) promptly for processing and reporting purposes.
• Utilize Found’s technology platform to keep patients engaged in their care journey between sessions.
• Stay informed on current nutrition topics.
• Credentialed as an RD/RDN with the CDR (active and in good standing).
• Licensed in multiple states, including a combination of: NY, IN, CA, PA, NC, GA, TX, SC, MD, OH, CT, MA, HI, SD, or MT. Candidates with licenses in additional states are encouraged to apply. Active NPI is required.
• Minimum of 2 years of independent practice experience, preferably in obesity medicine, cardiometabolic disease, or medication-assisted weight loss.
• Prior telemedicine experience is preferred.
• Excellent and empathetic oral and written communication skills.
• Ability to adapt and remain flexible while contributing to enhancing care delivery.
• Experience in obesity medicine and/or relevant certification is preferred.
• Candidates must be authorized to work and physically located in the U.S. for this position.
• Proficient in EMR documentation (ADIME notes) and secure provider-to-patient messaging; comfortable with quickly learning new clinical software.
• Must have reliable high-speed internet and a private, HIPAA-compliant workspace equipped with a professional-quality webcam and headset.
