Telehealth Provider – Urgent Care

This is a fully remote position, open to applicants in Connecticut, +2 more states.

📋 Description

• Assess, diagnose, and provide treatment to patients via phone and video telehealth consultations.

• Gather focused patient histories and review relevant medical documentation.

• Create treatment plans based on evidence-based practices.

• Order and analyze laboratory and diagnostic tests when clinically necessary.

• Prescribe medications following state and federal guidelines.

• Collaborate with colleagues and specialists to enhance patient outcomes.

• Manage referrals and escalate to in-person care when clinically appropriate.

• Provide compassionate and efficient care while ensuring an outstanding patient experience.

• Address common urgent care issues such as upper respiratory infections, asthma and COPD, skin rashes and infections, allergic reactions, eye infections and injuries, headaches, chest pain and cardiac concerns, as well as musculoskeletal injuries, sprains, and fractures.

• Contribute to an established 24/7 virtual care program and assist in broadening access throughout the Yale New Haven Health network.

⛳️ Requirements

• Prior telehealth experience is mandatory.

• Active licenses in Connecticut, New York, and Massachusetts are required.

• Possession of a Controlled Substance Registration.

• Federal DEA Registration is necessary.

• Board Certification as an Advanced Practitioner is essential.

• ACLS Certification is required.

• PALS Certification is necessary.

• Demonstrated strong independent clinical decision-making abilities.

• Capacity to excel in a fast-paced virtual care environment.

• Candidates must live within Connecticut, New York, or Massachusetts.

• Strong clinical knowledge and experience in managing common urgent care situations across all age demographics.

• Full-time commitment consisting of four 9-hour shifts each week, which includes morning, daytime, evening, and weekend coverage.
