
Telehealth Preventative Primary Care Advanced Practice Provider – NP/PA

This is a fully remote position, open to applicants in Texas.
• Primarily conduct telehealth video consultations, with some limited and occasional in-home visits to members within your local vicinity.
• Execute thorough assessments and deliver evidence-based care that emphasizes prevention, chronic condition management, medication optimization, and health maintenance.
• Recognize and address any gaps in care.
• Collaborate with both external and internal team members involved in patient care.
• Contribute to personalized, holistic care plans for each individual.
• Leverage integrated technology and AI-driven workflows to minimize administrative tasks and enhance meaningful interactions with members.
• Engage with members and caregivers with empathy and clarity, building trust and confidence.
• Work alongside primary care physicians, specialists, pharmacists, nurses, care coordinators, caregivers, and families to ensure coordinated, member-focused care.
• A valid Nurse Practitioner (APRN/NP) or Physician Assistant (PA) license is required.
• At least 3 years of outpatient clinical experience, preferably in primary care, family medicine, internal medicine, or geriatrics.
• An active and clear NP/PA license is essential.
• Must be open to acquiring additional licenses if required within the first 90 days, with support and reimbursement provided.
• Current BLS certification must be obtained by the time of hire.
• Comfortable providing care through video telehealth platforms.
• Proficient in using electronic documentation systems.
• Knowledge of managed care models, including STARS/HEDIS, and the identification of care gaps.
• Experience in conducting comprehensive or preventive care visits with Medicare populations.
• Background in supporting older adults and/or individuals with complex medical or social needs.
