Telehealth Preventative Primary Care Advanced Practice Provider – NP/PA

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Primarily conduct telehealth video consultations, with occasional in-home visits to members in your local vicinity.

• Execute thorough assessments and deliver evidence-based care concentrating on prevention, chronic condition management, medication optimization, and health maintenance.

• Identify and tackle gaps in care.

• Collaborate with both external and internal care team members.

• Participate in creating individualized, holistic care plans.

• Leverage integrated technology and AI-enhanced workflows to lessen administrative tasks and enhance meaningful interactions with members.

• Communicate with empathy and clarity, building trust and confidence with members and their caregivers.

• Work together with primary care providers, specialists, pharmacists, nurses, care coordinators, caregivers, and families to provide coordinated, member-focused care.

⛳️ Requirements

• Valid Nurse Practitioner (APRN/NP) or Physician Assistant (PA) license.

• Over 3 years of outpatient clinical experience, preferably in primary care, family medicine, internal medicine, or geriatrics.

• An active and clear NP/PA license.

• Willingness to obtain additional licenses if required within the first 90 days.

• Current BLS certification at the time of hire.

• Comfortable delivering care through video telehealth and utilizing electronic documentation systems.

• Knowledge of managed care models, including STARS/HEDIS and care gap identification.

• Experience in conducting comprehensive or preventive care visits with Medicare populations.

• Background in supporting older adults and/or individuals with complex medical or social needs.
