Telehealth Nurse Practitioner

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Perform thorough health assessments via telehealth.

• Record HCC risk adjustments during patient consultations.

• Address and resolve HEDIS (quality measures) care gaps.

• Evaluate medical history, current medications, and preventive requirements.

• Accurately document visits utilizing ICD-10 and CPT II codes.

• Provide clear care plans along with follow-up instructions.

• Ensure precise documentation for quality improvement programs.

⛳️ Requirements

• Valid Nurse Practitioner license in Missouri.

• Over 1 year of experience in Family Medicine or Internal Medicine.

• Familiarity with Medicare and Medicaid patient populations.

• Knowledge of HEDIS and risk adjustment processes.

• Required enrollment as a provider for Medicare and Medicaid.

• Proficient in delivering care via telehealth technologies.
