System Clinical Pharmacy Specialist – OPAT/COpAT

This is a fully remote position, open to applicants in Kentucky.

📋 Description

• Act as the subject matter expert for outpatient parenteral antimicrobial therapy (OPAT) and complex outpatient antimicrobial therapy (COpAT) services throughout the health system.

• Collaborate with antimicrobial stewardship leadership, infectious disease specialists, ambulatory leadership, care management, infusion services, and interdisciplinary stakeholders.

• Develop, implement, maintain, and optimize workflows, clinical pathways, documentation standards, monitoring programs, and quality initiatives for OPAT/COpAT.

• Ensure safe transitions of care for patients discharged on antimicrobial therapy.

• Evaluate antimicrobial regimens, laboratory monitoring plans, vascular access considerations, culture data, and recommendations for therapy duration.

• Monitor patients for therapeutic effectiveness, adverse drug reactions, laboratory abnormalities, drug interactions, adherence issues, and complications related to lines.

• Identify and address antimicrobial-related complications, dosing errors, medication interactions, readmission risks, and therapy failures.

• Maintain OPAT/COpAT dashboards, quality metrics, operational metrics, and outcome reporting.

• Assist with regulatory and accreditation preparedness.

• Optimize reporting, documentation, Epic order sets, discharge workflows, laboratory monitoring documentation, and clinical decision support tools.

• Conduct reviews of antimicrobial regimens and support formulary management, medication-use evaluations, quality improvement initiatives, formulary reviews, policies, protocols, and order sets.

• Provide clinical consultation on outpatient antimicrobial therapy, adverse drug reactions, laboratory interpretation, vascular access, therapeutic drug monitoring, and transitions of care.

• Aid in managing antimicrobial shortages and identifying therapeutic alternatives.

• Coordinate and deliver education, presentations, competency assessments, educational materials, and process updates.

• Precept pharmacy students and residents, as applicable.

• Engage in local, state, and national professional organizations and initiatives.


⛳️ Requirements

• Bachelor’s degree in pharmacy or Doctorate of Pharmacy is required.

• A license from the Kentucky Board of Pharmacy and the Indiana Board of Pharmacy, or the willingness and eligibility to obtain within 6 months of hire.

• The Kentucky license must indicate that the individual is a preceptor.

• Must be board certified in the area of expertise, or able to achieve this certification within 12 months of hire.

• Board Certified Infectious Diseases Pharmacist (BCIDP) is required.

• Completion of a PGY-2 ASHP-accredited Pharmacy Residency in a related area of expertise; OR completion of a PGY-1 ASHP-accredited Pharmacy Residency and at least 3 years of experience in a related area of expertise along with BPS board certification at the time of application; OR a minimum of 5 years of experience in a related area of expertise and BPS board certification at the time of application.

• Proficiency in infectious diseases pharmacotherapy, outpatient antimicrobial management, transitions of care, and principles of antimicrobial stewardship.

• Familiarity with workflows involving inpatient units, ambulatory clinics, infusion services, emergency departments, urgent care settings, and telehealth services.

• Knowledge of Epic support and clinical decision support tools.

• Involvement in infectious diseases, antimicrobial stewardship, ambulatory pharmacy practice, and OPAT/COpAT professional organizations and initiatives.
