
Support Transition and Reentry Specialist

This is a fully remote position, open to applicants in North Carolina.
• The STAR Specialist offers support to participants enrolled in the Trillium Support Transition and Reentry (STAR) program.
• STAR facilitates care coordination before and after release until individuals transition to ongoing Care Management services and support.
• The STAR Specialist informs members and guardians throughout the transitional phase on how to effectively navigate the criminal justice system, as well as behavioral, physical, and social services, including community-based services and supports.
• This role involves collaborating with community resources and services to address the identified needs of recipients reintegrating into the community and overcoming specific reentry barriers such as access to insurance, disability benefits, transportation, food, clothing, and safe housing.
• Ensure that recipients are thoroughly connected and provide a warm handoff to facilitate necessary treatment and support.
• Meet with Justice-Involved members both in Correctional Facilities and in the community to support care management activities.
• Engage with members who qualify for the Transitions to Community Living (TCL) special needs population to provide education on permanent supportive housing, resources, services, and additional community support.
• Assist with transition planning and ensure delivery in the most effective manner, focusing on achieving optimal quality outcomes.
• Employ person-centered planning methods and strategies to collect information, understand the members and their legally responsible persons, and adequately address the specific needs of each individual.
• High School Diploma or GED is required.
• Must have at least one (1) year of lived experience with a behavioral health condition, substance use, and/or involvement in the justice system.
• North Carolina Peer Support Specialist Certification is mandatory.
• A valid driver’s license is required.
• Candidates must reside within 75 miles of Trillium’s Mid State Region, which includes Anson, Guilford, Montgomery, Randolph, and Richmond counties.
• Ability to travel within the catchment area as necessary is required.
• Preference will be given to applicants who have personal experience with the criminal justice system, incarceration, or recovery from mental health/substance use issues, as well as education, training, and work experience that demonstrates the ability to engage with a diverse range of individuals from various socio-economic, cultural, ethnic, educational, and other backgrounds.
