Support Coordinator

This is a fully remote position, open to applicants in Australia.

📋 Description

• Assist NDIS participants in effectively navigating their plans.

• Facilitate connections between participants and suitable support services.

• Address challenges and empower participants to enhance their choice, confidence, and independence.

• Establish trustworthy relationships with participants, their families, providers, and other stakeholders.

• Coordinate efforts among stakeholders to align with participant goals.

• Oversee various support requirements, manage competing priorities, and engage multiple stakeholders.

• Identify practical solutions to participant-specific challenges.

• Organize and prioritize a caseload effectively.

• Track participant support issues as circumstances evolve.

⛳️ Requirements

• Proven experience in Support Coordination.

• Strong comprehension of the NDIS framework.

• Familiarity with local communities and service landscapes.

• Well-established networks and relationship-building skills with participants, families, providers, and stakeholders.

• Capacity to manage intricate support needs, competing priorities, and multiple stakeholders.

• Resourceful and practical problem-solving abilities.

• Competence in confidently managing and prioritizing a caseload while ensuring task completion.

• Relevant qualification (Cert III or above) in disability, mental health, allied health, community services, or another human services discipline is preferred.

• Experience in supporting participants with more complex needs is advantageous.
