This is a fully remote position, open to applicants in United Kingdom.

📋 Description

• Deliver a dedicated service to ILS Clients focusing on the coordination, rehabilitation, care, and support of individuals with complex clinical needs.

• Offer a service designed to enhance Client independence and elevate their quality of life while considering safety concerns.

• Responsibilities as a Case Manager will encompass: Conducting assessments of clients’ functions and needs.

• Creating precise and comprehensive formal reports that include the identification of time required to complete tasks.

• Setting and reviewing objectives.

• Conducting risk assessments and recognizing safeguarding issues.

• Acting as an advocate for clients.

• Providing support to clients and their families.

• Collaborating with professionals and other agencies on behalf of the client.

• Recruiting and overseeing Care Teams.

• Commissioning and coordinating multi-disciplinary teams.

• Completing and preparing all necessary documentation related to the Client as mandated by the Care Quality Commission.

• Attending court as necessary.


⛳️ Requirements

• Must be a qualified healthcare or social care professional (e.g. Occupational Therapist, Physiotherapist, Nurse, or Social Worker) with current HCPC, NMC, or SWE registration and an up-to-date CPD portfolio.

• Significant experience working with Children and Young People aged 0-25 and their families.

• A thorough understanding and application of the principles of the Mental Capacity Act 2005.

• Experience in conducting complex assessments, risk evaluations, and supporting clients with intricate needs.

• Strong knowledge of the UK health and social care system as well as rehabilitation principles.

• Demonstrated ability to work independently, juggle multiple priorities, and meet deadlines.

• Experience in community-based settings while maintaining professional boundaries and achieving favorable client outcomes.

• Exceptional written and verbal communication skills, including proficiency in report writing and case documentation.

• Proficient in Microsoft Office and comfortable using digital systems.

• Capability to lead, motivate, and coordinate multidisciplinary teams.

• Willingness to travel and adapt working arrangements, with access to personal transportation.
