Social Worker – Population Health

This is a fully remote position, open to applicants in Ohio, +4 more states.

📋 Description

• Deliver social work services to patients, families, and significant others by providing counseling, emotional support, environmental assistance, crisis intervention, and group leadership.

• Conduct psychosocial evaluations to assess emotional, social, and community strengths and challenges.

• Participate as a key member of interdisciplinary treatment teams guided by physicians.

• Develop and implement comprehensive treatment plans utilizing evidence-based practices and interventions.

• Screen, educate, and offer therapy and interventions for issues such as substance abuse, depression, anxiety, thought disorders, trauma, and autism spectrum presentations.

• Provide consultative services to healthcare team members while staying informed about pertinent medical and legal matters.

• Educate patients and families on adapting to diagnoses, illnesses, treatments, and life situations.

• Represent the clinical social work perspective and act as a liaison among patients, families, community agencies, and resources.

• Create discharge plans from admission through the duration of stay.

• Lead cognitive/psychoeducational groups, individual solution-focused brief therapy sessions, and crisis interventions.

• Update families, support persons, and caregivers, and facilitate family meetings.

• Assist with referrals to various placement options, including personal care homes, nursing homes, skilled rehabilitation, assisted living, and group homes.

• Document assessments, plans, interactions, and interventions in compliance with organizational policies.

• Maintain electronic medical records and engage in staff, committee, department, and administrative meetings.

⛳️ Requirements

• Master's degree in social work or a related discipline.

• Current social worker licensure as mandated by the state where services are provided.

• WV: LGSW, LCSW, or LICSW through the West Virginia Board of Social Work.

• PA: No state licensure necessary.

• OH: LSW, LISW, or LISW-S through the Ohio Board of Social Work.

• MD: LMSW, LCSW, or LCSW-Clinical through the Maryland Board of Social Work.

• Must acquire master's-level social work licensure in Ohio within six months of employment.

• A minimum of two years of social work experience is preferred.

• Outstanding verbal and written communication abilities.

• Capability to collaborate effectively with healthcare professionals.

• Basic computer literacy and proficiency in standard office software.

• Ability to endure prolonged periods of standing or sitting.

• Familiarity with the Northern Panhandle of West Virginia and its community resources is preferred.
