Social Worker

This is a fully remote position, open to applicants in Ohio, +4 more states.

📋 Description

• Deliver social work services to patients, families, and significant others through counseling, emotional support, environmental assistance, crisis intervention, and group leadership.

• Perform psychosocial assessments for patients and their families.

• Collaborate as a member of the interdisciplinary treatment team led by a physician.

• Create and execute comprehensive treatment plans utilizing evidence-based treatments and interventions.

• Screen, educate, and provide therapeutic interventions for issues such as substance abuse, depression, anxiety, thought disorders, trauma, and autism spectrum presentations.

• Offer consultative services to healthcare team members.

• Inform patients and families about adapting to diagnoses, illnesses, treatments, and life circumstances.

• Engage in multidisciplinary healthcare teams and act as the clinical social work liaison.

• Formulate discharge plans from the time of admission through the duration of stay.

• Connect with community agencies and resources.

• Lead cognitive/psychoeducational groups, individual solution-focused brief therapy, and crisis intervention sessions.

• Provide updates to families, support persons, and caregivers; facilitate family meetings.

• Coordinate referrals to various placement options.

• Record assessments, plans, interactions, and interventions.

• Maintain electronic medical records and take part in staff, committee, department, and administrative meetings.

⛳️ Requirements

• Master’s degree in social work.

• An active West Virginia social work license at the time of hire.

• Must secure master's-level social work licensure in Maryland within six months of employment.

• Current social worker licensure as mandated by the state where services are provided.

• A minimum of two years’ experience in social work (preferred).

• Certification in Case Management (preferred).

• Exceptional verbal and written communication skills.

• Ability to collaborate effectively with healthcare professionals.

• Basic computer proficiency and capability to use standard office software.

• Capability to endure prolonged periods of standing or sitting.

• Flexibility to work hours that include weekends and holidays in inpatient settings.
