SNP Engagement Specialist

This is a fully remote position, open to applicants in United States.

📋 Description

• Collaborate with the SNP Clinical team to ensure continuous care coordination for members.

• Assist in meeting the compliance standards set forth by CMS SNP Model of Care (MOC).

• Support members in navigating the completion of Health Risk Assessments.

• Conduct prompt follow-ups with members and/or their appointed caregivers.

• Provide education to members regarding DSNP supplemental benefits and help them access these resources.

• Assist in arranging appointments for PCP visits and preventive screenings.

• Maintain accurate documentation to facilitate the coordination of members' care requirements.

⛳️ Requirements

• High School diploma or its equivalent.

• At least one (1) year of experience in a healthcare or managed care environment.

• A minimum of one (1) year of customer service experience.

• Exceptional written and verbal communication skills.

• Solid understanding of CMS regulatory and contractual documents.

• Proficient in Microsoft Office applications.
