This is a fully remote position, open to applicants in Illinois, +2 more states.
• Transform the understanding, prediction, and delivery of patient and population needs using an AI-driven orchestration engine.
• Offer strategic and operational leadership across the enterprise for integrated care coordination, navigation, patient flow, and access.
• Develop core capabilities, optimize resources, scale solutions, enhance impact, monetize platforms, and achieve established KPIs.
• Improve inpatient and ambulatory care management, care transitions, care navigation, coordination, access, and patient flow.
• Establish platforms, processes, and structures that unify inpatient, ambulatory, and virtual care operations.
• Collaborate with physicians, nursing staff, IT, Virtual Health, Clinical Transformation, operations, strategy, and divisional leadership.
• Design, develop, and execute comprehensive orchestration for Care Management and PACT.
• Set system standards and enhance performance through data and technology utilization.
• Act as the operational architect and primary liaison for evaluating Mission Control outcomes related to technology, algorithms, care pathways, and associated changes.
• Lead the enterprise "mission control" for care coordination, patient flow, and access throughout the care continuum.
• Convert enterprise and system strategy into integrated operating frameworks, priorities, and performance expectations.
• Establish governance structures, decision rights, escalation pathways, and standardized processes for patient progression and care coordination.
• Provide executive leadership for inpatient care management, ambulatory care management, care coordination, utilization management, and care transitions.
• Facilitate integration across care settings to minimize fragmentation, enhance outcomes, and optimize length of stay, readmissions, and post-acute transitions.
• Oversee the PACT Call Center at the enterprise level and integrate access, triage, and navigation with clinical care management.
• Utilize digital tools, analytics, and workforce models to elevate access, responsiveness, and patient experience.
• Implement reliable personnel, processes, and technology that support an integrated, high-performing patient experience.
• Develop and operationalize financially sustainable care models.
• Apply advanced analytics and real-time data to forecast demand, manage capacity, and initiate proactive interventions.
• Collaborate with corporate stakeholders and clinical executives to ensure transparency, accountability, and alignment.
• Work with market, medical group, and hospital leadership to implement enterprise models.
• Drive system-wide change through matrixed leadership.
• Establish expectations, identify dependencies and redundancies, clarify ownership, and prioritize tasks.
• Lead and cultivate senior leaders across various portfolios and geographic locations.
• Foster a high-performing, accountable leadership team rooted in Advocate Health values.
• Master’s degree in healthcare administration, Business Administration, Nursing, Medicine, or a related field, or equivalent experience.
• At least 15 years of progressive leadership experience in healthcare, including enterprise or multimarket scopes.
• Proven track record of leading large, complex clinical or operational entities across multiple care environments.
• Extensive expertise in care management, patient flow, access, and clinical operations.
• Demonstrated ability to operate at the executive strategy level while ensuring disciplined execution.
• Experience in managing large, complex teams across various geographies and care settings.
• Capability to collaborate with physicians, nursing, IT, Virtual Health, Clinical Transformation, operations, strategy, and divisional leadership.
• Aptitude for translating enterprise strategy and design into coordinated execution at scale in a financially sustainable manner.
• Experience with advanced analytics and real-time data.
• Licensure, registration, and/or certification: none required.
• This is a remote-based position with a strong preference for candidates residing in NC, GA, IL, or WI.
• Some travel will be required.
