This is a fully remote position, open to applicants in Illinois.
• Provide support to all care teams throughout the region by conducting thorough and focused chart reviews.
• Promote efficient and effective medical care for patients.
• Collaborate with regional leaders, especially the Population Health Director and Senior CIS, to enhance daily huddle and CDR workflow efficiency.
• Access and examine HIE/EMRs to pinpoint potential suspects based on test outcomes.
• Determine the necessity for Medical Records requests from Blue Button and other payer data sources.
• Organize and lead monthly Rejected Suspect Reviews.
• Work alongside the Population Health Director to provide general review assistance.
• Identify and evaluate opportunities to enhance patient quality outcomes and reduce care costs.
• Analyze the prevalence of chronic conditions based on patient demographics and recognize intervention opportunities.
• Oversee clinical documentation efforts.
• Perform additional duties as assigned.
• Proficient and comfortable working with various technical systems, including Greenway, Canopy, and Excel.
• Ability to collaborate effectively across different teams and consider multiple perspectives to achieve a shared goal.
• Demonstrated experience and capability in building relationships with Providers.
• Skilled in sourcing and utilizing external resources, such as hospital records, Blue Button, and payer data, to ensure accurate retrospective and prospective documentation.
• Capable of thriving in a fast-paced, constantly evolving environment.
• Self-motivated with a strong degree of drive, initiative, and follow-through.
• Ability to create, plan, and execute both short-term and long-term goals.
• Outstanding interpersonal, written, and verbal communication abilities.
