This is a fully remote position, open to applicants in Kentucky.
• Evaluate and assess the needs of members to promote health and wellness outcomes.
• Collaborate with members, their families, and care teams to create and implement tailored care plans.
• Coordinate services and link members to resources that cater to both their clinical and social needs.
• Perform thorough assessments to pinpoint member needs, risks, and obstacles to care.
• Design and oversee member-focused care plans with specific measurable goals and outcomes.
• Track progress and modify interventions to ensure successful outcomes.
• Address social determinants of health, such as housing, transportation, and access to care.
• Employ evidence-based care pathways to inform clinical decision-making.
• Engage with members using coaching and motivational interviewing techniques.
• Travel to conduct in-person visits with members in various settings to facilitate holistic assessment and care coordination.
• Must reside in Western Kentucky, ideally within the surrounding counties of Caldwell, Lyon, Crittenden, Livingston, Hopkins, Christian, or Trigg.
• Active Registered Nurse (RN) license required.
• Bachelor's degree in nursing or a healthcare/human services-related field.
• A minimum of 2 years of experience as a case manager or care manager.
• Experience working with adult populations is essential.
• Familiarity with community health services, social services, and available resources.
• Proficient in Microsoft Office (Word, Outlook, Excel) and electronic medical records.
