This is a fully remote position, open to applicants in United States.
• Perform clinical evaluations and deliver disease- and therapy-related education to patients and their caregivers.
• Recognize and address needs concerning therapy usage, adherence, persistence, side effects, and disease-state issues.
• Refer patients to a pharmacist and/or inform the treating provider as clinically appropriate.
• Record clinical assessments, patient interventions, care coordination efforts, and clinical notes in patient files.
• Assist in the planning, execution, and maintenance of clinical programs and projects.
• Complete adverse event documentation and program-specific paperwork.
• Collaborate with pharmacists, clinical team members, manufacturers, client partners, provider offices, and clinics.
• Help with order creation and setup, payment options, copay assistance, order specifics, and shipment coordination.
• Maintain clinical expertise across various treatments, disease states, protocols, and specialty pharmacy services.
• Ensure adherence to accreditation standards, pharmacy policies, HIPAA, and relevant federal and state regulations.
• Promote safe and effective therapy usage, adherence, persistence, patient outcomes, quality, and program requirements.
• Perform additional duties as assigned.
• Associate Degree in Nursing (ADN) is mandatory.
• Bachelor of Science in Nursing (BSN) is preferred.
• Current, unrestricted licensure as a Registered Nurse in the state of residence is required.
• Capability to acquire additional single-state licenses as necessary.
• Unrestricted multi-state compact nursing license is preferred.
• A minimum of 2 years of experience in patient-centered care or disease management nursing is preferred.
• Actively participate in, adhere to, and support compliance program objectives.
• Ability to consistently engage cooperatively and respectfully with colleagues.
• Bilingual proficiency is required; specific languages are not mentioned.
• Residents of Alaska, Connecticut, Delaware, Hawaii, Idaho, Kansas, Kentucky, Maine, Massachusetts, Mississippi, Montana, New Hampshire, New Mexico, North Dakota, Rhode Island, South Carolina, South Dakota, Vermont, Washington DC, West Virginia, and Wyoming are not eligible for remote work.
