RN Case Manager, Health Plan

This is a fully remote position, open to applicants in North Dakota, +1 more state.

📋 Description

• Provides comprehensive case management and care coordination for assigned Health Plan members

• Develops individualized care plans that involve managing and directing members to suitable resources, services, and programs

• Conducts assessments of members, identifying gaps and necessary interventions

• Coordinates continuous care with physicians and the integrated care team

• Addresses the medical and behavioral health needs of members

⛳️ Requirements

• Bachelor's degree in nursing is mandatory

• Graduation from a nationally accredited nursing program is preferred

• The ideal applicant will hold an RN license from ND, SD, NE, or IA

• A Master's degree in counseling, social work, marriage and family therapy, or psychology is required

• A minimum of three years of nursing experience is essential

• At least five years of experience in behavioral health is required

• Familiarity with DSM V and ASAM criteria is necessary

• Current certification from the Commission for Case Manager Certification (CCMC) is preferred
