This is a fully remote position, open to applicants in Louisiana.

📋 Description

• Create tailored care plans that reflect the goals, preferences, and desired levels of involvement of both patients and caregivers.

• Conduct outbound calls to evaluate the current health status of patients.

• Recognize gaps or obstacles in treatment protocols.

• Facilitate care coordination for members and refer them to external resources as needed.

• Ensure that members transitioning from the hospital or emergency department receive appropriate services and resources, including medication reconciliation.

• Triage symptom-related calls according to approved protocols for a diverse patient population.

• Enroll patients and oversee alerts for various home monitoring initiatives.

• Provide general health information and guidance to public callers from approved sources by assessing their needs.

• Document all processes in the EPIC system for tracking and quality assessment purposes.

• Offer patient education to support self-management of health and wellness.

• Inform enrolled members about both chronic and acute disease processes.

• Collaborate with LCMC Health care partners, leadership, and/or physicians to address clinical inquiries, concerns, strategies, and care plans aimed at achieving quality and cost management goals.

• Exhibit enthusiasm, energy, and a commitment to delivering high-quality work.


⛳️ Requirements

• Valid nursing license to practice in Louisiana.

• Minimum of 2 years of experience in professional nursing or care management.

• Experience in Utilization Review and/or as a Care Manager is preferred.

• Required experience in an Acute Hospital setting.

• Familiarity with Epic & InterQual systems is preferred.
