Risk Care Management – Project Manager

This is a fully remote position, open to applicants in District of Columbia, +3 more states.

📋 Description

• Deliver comprehensive project management and implementation assistance for the design, launch, administration, evaluation, and scaling preparation of the Elevated Risk Care Management (ERCM) Pilot.

• Act as the sole contractor point of contact for the Veterans Health Administration (VHA) concerning subject matter expert support and contractor performance.

• Plan, initiate, manage, execute, monitor, and conclude project activities throughout the pilot lifecycle.

• Create and uphold project plans, schedules, milestones, dependencies, deliverables, and action-item tracking.

• Coordinate efforts between Government and contractor teams, pilot sites, clinical teams, stakeholders, and subject matter experts.

• Track risks, issues, resource constraints, dependencies, schedule impacts, and overall project performance.

• Oversee ERCM Pilot implementation across three designated sites, including onboarding, training, stakeholder engagement, workflows, and launch activities.

• Assist with evaluation, data collection, dashboards, status reports, executive briefings, and evidence-based assessments of pilot effectiveness.

• Develop project plans, implementation plans, technical documentation, workflow and training materials, evaluation summaries, and transition documents.

• Aid in transition planning, implementation toolkits, national expansion, ongoing performance monitoring, and continuous improvement.

⛳️ Requirements

• Bachelor's degree (BA/BS) from an accredited institution in business administration, healthcare administration, health services, public health, project management, or a related field.

• At least four years of relevant project/program management experience.

• Project Management Professional (PMP) certification.

• Preferred experience in supporting the Veterans Health Administration (VHA), Department of Veterans Affairs (VA), or another large integrated healthcare system.
