This is a fully remote position, open to applicants in New Jersey.
• In the role of Resource Specialist Care Manager, you will assist families of children with complex medical conditions by providing emotional support, coordinating care, and guiding them through the healthcare system.
• Conduct comprehensive assessments, focusing on the current medical, behavioral, and intricate social needs.
• Identify obstacles to care, including food insecurity, unstable housing, transportation issues, and limitations related to insurance.
• Work collaboratively with families and multidisciplinary teams to set personalized care objectives.
• Create and sustain an integrated care plan that addresses medical, behavioral, and social requirements.
• Ensure prompt follow-up on referrals, evaluations, and treatment recommendations.
• Educate families on how to navigate local resources and agencies.
• Provide emotional support and guidance during challenging times for families.
• Advocate for resources and the elimination of barriers, connecting families with available assistance.
• Cultivate and sustain relationships with community organizations that support children facing medical and behavioral health challenges.
• Stay updated on new programs and resources relevant to the population served.
• Represent the organization at community events, health fairs, and resource meetings.
• Engage in regular consultations with an interdisciplinary team to monitor patient progress.
• Carry out additional duties as assigned.
• Master's degree with major coursework in social work, community/social services, and/or case management.
• LMSW is strongly preferred.
• Licensure/certification preferred (LMSW, CCMC, C-SWCM).
• 2-3+ years of experience in social service case management is required.
• A minimum of 1 year of experience working with the pediatric population is required.
• Knowledge and application of motivational interviewing practices and/or techniques.
• Comfort in working with acute populations and familiarity with school and community resources is highly preferred.
• Experience providing telehealth services and engaging with families in a remote setting.
• Bilingual Spanish is strongly preferred.
