This is a fully remote position, open to applicants in Canada.
• Assess the necessary rehabilitation evaluations to further gauge the Participant's potential for improvement, clarify their rehabilitation requirements, and coordinate as needed.
• Ensure that services and plans are based on individual needs, focused on goals, and aligned with program mandates aimed at enhancing the six functional domains of well-being while minimizing or eliminating barriers to reintegration, ensuring they are safe, cost-effective, and of high quality.
• Create detailed Rehabilitation Plans for program Participants in collaboration with the Participant, their family or caregiver, Rehabilitation Service Professionals (RSPs), and the Veterans Affairs Canada (VAC) Case Manager.
• Serve as the service integrator, working in partnership with medical, psycho-social, and vocational RSPs who provide services to participants.
• Empower Participants to explore solutions by investigating community-based interventions and resources to achieve their desired results.
• Recognize barriers or risk factors that may lead to suboptimal outcomes and proactively devise and implement strategies.
• A bachelor’s degree in Psychology, Counselling, Rehabilitation, Social Sciences, Health Services, Human Services, or a related discipline, along with one of the following: a health professional designation or Certified Vocational Rehabilitation Professional (CVRP or CVRP-F).
• At least three (3) years of experience in the direct delivery of medical, psycho-social, or vocational rehabilitation services (or a combination thereof).
• Experience working with individuals facing mental or physical health challenges that hinder their daily lives.
• Employment is contingent upon obtaining federal security clearance – Reliability Status.
• Willingness to travel throughout the region as needed.
• Bilingualism (French/English) is considered an asset.
• Experience collaborating with employers, community resources, and training institutions.
• Current and former members of the Canadian Armed Forces (CAF) are encouraged to apply.
