This is a fully remote position, open to applicants in Australia.
• Assess the necessary rehabilitation evaluations to further analyze the Participant's potential for improvement, clarify rehabilitation requirements, and coordinate as needed.
• Ensure that services and plans are tailored to individual needs, focused on goals, and align with program objectives aimed at enhancing the six functional domains of well-being while minimizing or removing barriers to re-establishment, ensuring safety, cost-effectiveness, and high quality.
• Create a comprehensive Rehabilitation Plan for program Participants in collaboration with the Participant, their family/caregiver, Rehabilitation Service Professionals (RSPs), and the Veterans Affairs Canada (VAC) Case Manager.
• Serve as the service integrator by collaborating on equal footing with medical, psycho-social, and vocational RSPs providing services to participants.
• Empower Participants to engage in problem-solving by exploring community-based interventions and resources to attain desired outcomes.
• Identify obstacles or risk factors that lead to suboptimal results and proactively develop and execute strategies.
• A bachelor’s degree in Psychology, Counselling, Rehabilitation, Social Sciences, Health Services, Human Services, or a related discipline.
• A designation as a health professional, or as a Certified Vocational Rehabilitation Professional (CVRP or CVRP-F).
• A minimum of three (3) years of experience in the direct delivery of medical, psycho-social, or vocational rehabilitation services, or a combination thereof.
• Experience working with individuals facing mental or physical health challenges that create barriers in their lives.
• Employment is contingent upon obtaining federal security clearance – Reliability Status.
• Willingness to travel across the region as necessary.
• Preferred Criteria: Bilingual proficiency in French and English is considered an asset.
• Experience collaborating with employers, community resources, and training institutions.
