This is a fully remote position, open to applicants in Canada.
⢠Identify necessary rehabilitation assessments to evaluate the Participantās potential for improvement and clarify their rehabilitation requirements, coordinating as necessary.
⢠Ensure that services and plans are tailored to individual needs, focused on goals, and aligned with program mandates aimed at enhancing the six functional domains of well-being while minimizing or eliminating barriers to re-establishment, ensuring safety, cost-effectiveness, and high quality.
⢠Create comprehensive Rehabilitation Plans for program Participants in collaboration with the Participant, their family/caregiver, Rehabilitation Service Professionals (RSPs), and the Veterans Affairs Canada (VAC) Case Manager.
⢠Serve as the service integrator, working in partnership with medical, psycho-social, and vocational RSPs who deliver services to participants.
⢠Encourage Participants to engage in problem-solving by exploring community-based interventions and resources to meet their desired outcomes.
⢠Recognize barriers or risk factors that may lead to less than optimal outcomes and proactively devise and implement strategies.
⢠A bachelorās degree in Psychology, Counselling, Rehabilitation, Social Sciences, Health Services, Human Services, or a related area, along with one of the following: a health professional designation; or Certified Vocational Rehabilitation Professional (CVRP or CVRP-F).
⢠At least three (3) years of experience in directly providing medical, psycho-social, or vocational rehabilitation services (or a combination of these).
⢠Experience in working with individuals facing mental or physical health challenges that create barriers in their lives.
⢠Employment is conditional upon obtaining federal security clearance ā Reliability Status.
⢠Willingness to travel throughout the region as required.
⢠Bilingual proficiency in French and English is considered an asset.
⢠Experience collaborating with employers, community resources, and training institutions.
