Registered Triage Nurse

This is a fully remote position, open to applicants in Florida.

📋 Description

• The Triage Registered Nurse is tasked with recognizing and coordinating the needs of patients and their families, offering preventive and supportive care to those facing terminal illnesses.

• Ensures continuity of care for patients and families outside of regular office hours by providing telephone triage and arranging after-hours patient care visits.

• Responds immediately to all incoming calls from patients and their families.

• Engages in resolving issues related to patient, family, or employee complaints.

• Provides necessary updates to the Executive Director or Director On-Call as needed.

• Evaluates the physical, psychological, social, and spiritual needs of patients and families within 24 hours of their admission.

• Accurately documents patient and family services within 24 hours of the visit and ensures the electronic medical record, including the medication profile, is kept up to date.

• Collaborates with the Interdisciplinary team to coordinate care for patients and their families, utilizing all available resources.

⛳️ Requirements

• Associate of Science in Nursing (ASN) is required; a Bachelor of Science in Nursing (BSN) is preferred.

• Must hold a clear and active Registered Nurse license to practice in the state of Florida.

• 1-2 years of nursing experience is preferred.

• Experience in hospice nursing is preferred.

• Certification in Hospice & Palliative Medicine (CHPN) is preferred and must be obtained within two years of employment.

• Proficiency in using computer office and clinical software is required.

• Ability to read, write, and comprehend the English language is necessary.
