This is a fully remote position, open to applicants in Florida.
β’ The Triage Registered Nurse is tasked with recognizing and coordinating the needs of patients and their families, offering preventive and supportive care to those facing terminal illnesses.
β’ Ensures continuity of care for patients and families outside of regular office hours by providing telephone triage and arranging after-hours patient care visits.
β’ Responds immediately to all incoming calls from patients and their families.
β’ Engages in resolving issues related to patient, family, or employee complaints.
β’ Provides necessary updates to the Executive Director or Director On-Call as needed.
β’ Evaluates the physical, psychological, social, and spiritual needs of patients and families within 24 hours of their admission.
β’ Accurately documents patient and family services within 24 hours of the visit and ensures the electronic medical record, including the medication profile, is kept up to date.
β’ Collaborates with the Interdisciplinary team to coordinate care for patients and their families, utilizing all available resources.
β’ Associate of Science in Nursing (ASN) is required; a Bachelor of Science in Nursing (BSN) is preferred.
β’ Must hold a clear and active Registered Nurse license to practice in the state of Florida.
β’ 1-2 years of nursing experience is preferred.
β’ Experience in hospice nursing is preferred.
β’ Certification in Hospice & Palliative Medicine (CHPN) is preferred and must be obtained within two years of employment.
β’ Proficiency in using computer office and clinical software is required.
β’ Ability to read, write, and comprehend the English language is necessary.

β’ Engage in hospice-related activities such as Memorial Services and Community Education, as requested by the Team Manager.
β’ Provide regular orientation to new employees.
β’ Ensure clear communication with Interdisciplinary Teams.
