This is a fully remote position, open to applicants in Oregon.
• Conduct telephonic outreach to members to support the closure of HEDIS and Stars measure gaps and facilitate re-engagement.
• Deliver evidence-based patient education focused on engagement and recommended health screenings.
• Employ a trauma-informed, member-centered approach.
• Assist members in navigating the continuum of care based on their clinical or medical requirements.
• Make referrals to community partners when appropriate.
• Perform chart reviews for regulatory compliance and quality improvement initiatives.
• Utilize EMR systems or PDF files to carry out chart abstractions and input data into HEDIS or other specialized software.
• Maintain a 95% accuracy rate in data abstractions and entries, validated through peer reviews.
• Collaborate effectively with CareOregon clinics, team members, and staff.
• Complete required ongoing HEDIS training and assessments.
• Engage in additional assigned projects.
• Conduct work in alignment with CareOregon’s mission, vision, values, policies, compliance standards, and strategic objectives.
• Must hold a current unrestricted Registered Nurse license in the State of Oregon.
• At least 3 years of experience as an RN in the healthcare sector is required.
• Understanding of or a willingness to learn about current clinical practices, chronic diseases, health-screening guidelines, and health maintenance.
• Capability to utilize a trauma-informed approach for member-centric assistance.
• Proficient in extracting clinical information from medical records, accurately documenting it in HEDIS or specialized software, and saving supporting documents.
• Strong critical thinking abilities.
• Excellent attention to detail.
• Proficient in PDF, MS Outlook, Word, Excel, and EMR systems.
• Ability to learn and adapt to new EMR systems, software updates, and changing specifications.
• Commitment to maintaining strict confidentiality of protected health information.
• Skills in prioritizing tasks in a remote work environment.
• Capacity to manage multiple responsibilities and switch tasks as necessary.
• Effectiveness in working with diverse individuals and groups.
• Ability to meet regulatory deadlines under pressure.
• Strong written and verbal communication skills.
• Capability to see, read, and perform repetitive finger and wrist movements for at least 6 hours per day.
• Ability to stand and sit for a minimum of 3–6 hours daily.
• Ability to speak clearly, and perform tasks such as bending, lifting, carrying, pushing, pulling, and/or pinching small objects for at least 1–3 hours per day.
