Registered Nurse Case Manager – LTSS

This is a fully remote position, open to applicants in New Jersey.

📋 Description

• Perform in-home, face-to-face evaluations for members who are enrolled in:

• Managed Long-Term Services and Supports (MLTSS)

• Dual Special Needs Programs (D-SNP/FIDE)

• Assess member requirements and coordinate services such as:

• Adult/pediatric medical day care

• Personal care assistant services

• Nursing facility placement

• Enrollment in the MLTSS program

• Collaborate with members, caregivers, primary care providers (PCPs), and interdisciplinary teams to create and execute tailored care plans.

• Authorize services in accordance with program policies and facilitate safe transitions of care.

• Accurately and promptly document all case activities in the electronic health record.

• Utilize strong clinical judgment and critical thinking to address complex care needs.

• Provide mentorship to new team members once proficiency has been achieved.

⛳️ Requirements

• Valid, unrestricted RN license in New Jersey

• A minimum of 3 years of clinical practice experience

• At least 3 years of experience in home health, hospice, or a comparable environment

• Capability to manage pediatric and medically intricate cases

• Residency within Passaic County, NJ

• Successful completion of the NJ Choice Certification, which includes:

• o A minimum exam score of 80%

• o Completion of state training modules

• o Field mentoring component

• Must have reliable transportation and be prepared to travel up to 75% of the time.
