Registered Nurse, Care Manager

This is a fully remote position, open to applicants in New York.

📋 Description

• Deliver care management for a designated group of health plan members.

• Develop Person-Centered Service Plans (PCSP) tailored to individual needs.

• Ensure coordination of services and maintain care documentation in accordance with health plan standards.

• Conduct assessment, planning, facilitation, and advocacy through a collaborative interdisciplinary approach.

• Execute quality and cost-effective care plans based on UAS-NY assessments.

• Work in partnership with members, families, healthcare teams, medical providers, and community-based programs.

• Manage a caseload of members within a managed care framework.

• Complete all regulatory documentation accurately, clearly, and in a timely manner.

• Oversee health plan support personnel, including LPNs, Community Health Workers, and Care Coordination staff.

• Provide training and guidance to support staff members.

• Engage in departmental and staff meetings, as well as mandated training sessions.

• Comply with state and federal privacy regulations, including HIPAA.

• Maintain communication and collaboration with members, families, health plan staff, and providers.

• Serve as a liaison and advocate for members.

• Exhibit cultural competence and respect for members' cultural health beliefs, language preferences, and support systems.

• Carry out additional responsibilities as assigned.

⛳️ Requirements

• Valid and current registration as a Registered Professional Nurse in New York State.

• An Associate degree is required.

• A Bachelor of Science in Nursing (BSN) is preferred.

• A minimum of one year of nursing experience is required.

• Experience in long-term care and/or managed care is advantageous.

• Familiarity with home care, Medicare, and Medicaid is a plus.

• Care Management Certification is preferred.

• Bilingual abilities are a plus.

• Capability to work independently and manage workload efficiently.

• Proficient in computer skills and basic knowledge of Microsoft Office programs.

• Strong communication skills, both in-person and virtually, as well as via telephone.

• Valid NYS driver's license with the ability to drive in various weather conditions.

• Willingness to learn and consistently use the organization’s software systems.

• Ability to perform tasks that may involve exposure to blood, body fluids, or tissues.

• Capacity to lift up to 20 pounds frequently and up to 50 pounds occasionally.
