This is a fully remote position, open to applicants in New York.
• Deliver care management services to a designated group of health plan members.
• Develop Person-Centered Service Plans (PCSP).
• Coordinate services and maintain documentation in accordance with health plan standards.
• Provide care management through assessment, planning, facilitation, and advocacy.
• Implement quality and cost-effective care plans utilizing available resources and UAS-NY assessments.
• Manage a caseload of members enrolled in managed care.
• Complete regulatory documentation accurately, concisely, and punctually.
• Coordinate support staff within the health plan, including LPNs, Community Health Workers, and Care Coordination teams.
• Offer guidance and education through a collaborative team approach.
• Participate in departmental meetings, staff gatherings, and training sessions.
• Comply with state and federal privacy regulations, including HIPAA.
• Communicate and collaborate effectively with members, families, health plan personnel, and providers.
• Serve as a liaison and advocate for members.
• Exhibit cultural competence concerning members' cultural health beliefs, language needs, and support systems.
• Execute additional duties as assigned.
• Current license and registration as a Registered Professional Nurse in New York State.
• An Associate degree is required.
• A minimum of one year of nursing experience is necessary.
• Experience in long-term care and/or managed care is advantageous.
• Familiarity with homecare, Medicare, and Medicaid is a plus.
• Care Management Certification is beneficial.
• Bilingual skills are an asset.
• Ability to work independently and manage workload effectively.
• Proficient in basic computer skills and knowledgeable in Microsoft Office programs.
• Strong communication skills, both in-person, virtually, and via phone.
• Valid NYS driver's license with the capability to drive in various weather conditions.
• Ability to learn and consistently utilize the organization’s software systems.
• Capability to meet physical demands, including lifting up to 20 pounds frequently and up to 50 pounds occasionally.
• Tasks may involve exposure or potential exposure to blood, body fluids, or tissues.
• Employment is contingent upon receiving negative results from a pre-hire drug screen.
