This is a fully remote position, open to applicants in Illinois.
• This role involves overseeing medical management and health education initiatives for clients enrolled in government healthcare programs.
• Responsibilities include collecting, analyzing, and delivering data for regulatory reporting purposes.
• This position acts as the company representative to members.
• Registered Nurse (RN) with 3 years of direct clinical experience in a healthcare setting, or a Licensed Professional Counselor (LPC) or Licensed Master Social Worker (LMSW) with 2 years of clinical practice to qualify for LPC or LMSW licensure.
• Must possess a current, valid, and unrestricted license in the state of operation (or through reciprocity). For compact licensees relocating permanently to the state of operation, active and unrestricted RN licensure must be obtained within 90 days of employment.
• Additionally, 3 years of experience in wellness or managed care, particularly in presenting clinical matters to members and physicians.
• Familiarity with the health and wellness market and employer trends.
• Strong verbal and written communication skills, including the ability to discuss medical needs with members and collaborate with internal staff, management, external vendors, and community resources.
• Experience in analyzing medical data.
• Willingness and ability to travel within the designated territory.
• Proficiency in PC applications such as Word, Excel, and PowerPoint, along with database experience and familiarity with web-based applications.
• A minimum of 3 years of clinical experience.
• Experience in patient education.
• Background in Condition Management.
• Bilingual proficiency in English and Spanish.
• Experience in Transition of Care.
• Experience in managing complex or catastrophic cases.
• Certification in Case Management, Training, Project Management, or other nationally recognized healthcare certifications.
