This is a fully remote position, open to applicants in Nebraska.
• Provide exceptional, patient-focused Medical Nutrition Therapy (MNT) for individuals with chronic and complex conditions such as diabetes, cardiovascular disease, kidney disease, obesity, cancer, and gastrointestinal disorders.
• Conduct thorough virtual assessments and create personalized nutrition care plans that align with the needs, preferences, and goals of each patient.
• Develop nutrition prescriptions that are specific to clinical diagnoses and encourage sustainable behavior modifications.
• Record all patient interactions in the electronic health record (EHR) in a clear and timely manner.
• Work collaboratively with care teams, including physicians and health coaches, to enhance coordinated patient care.
• Engage in the development of content for education, outreach, and patient engagement initiatives.
• Participate in ongoing quality improvement efforts and innovate care delivery methods.
• Utilize current nutrition research and evidence-based practices in patient care.
• Uphold necessary professional credentials, licensing, and commit to ongoing education.
• Attend cross-functional meetings and contribute to strategic projects as appropriate.
• Current Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credentialed by the Commission on Dietetic Registration, in good standing AND a Master’s Degree in Nutrition or a related field preferred; OR a Bachelor's Degree with RD/RDN status achieved prior to the Master's degree requirement taking effect (January 1, 2024).
• Licensed to practice in the state of Nebraska.
• Exceptional communication abilities and a strong dedication to patient-centered care.
• Comfortable utilizing digital tools and platforms for virtual healthcare delivery.
• Capable of effectively engaging with diverse patient populations and various health conditions.
• Strong problem-solving, critical thinking, and organizational skills.
• A proactive, collaborative mindset and the ability to navigate ambiguity in a fast-paced setting.
• A passion for innovation, public health, and increasing access to nutrition care.
• Bonus If You Are: Credentialed with major health plans.
• Actively licensed in states other than Nebraska.
• Experienced in behavior change counseling, group education, or culinary nutrition.
• Comfortable participating in program design, content development, or digital tool utilization.
• Fluent in Spanish.
