This is a fully remote position, open to applicants in Florida.
• Perform thorough assessments of members' health conditions and risks utilizing clinical tools and data analysis.
• Create care plans tailored to members' health status, risks, educational needs, and support requirements.
• Detect quality-of-care concerns through designated channels.
• Employ motivational interviewing and influencing skills to enhance member engagement and encourage lifestyle and behavioral modifications.
• Offer coaching and support through active listening and interviewing techniques, empowering members to make independent choices for healthier lifestyles.
• Collaborate with members and/or attending physicians to develop and oversee care plans.
• Adhere to internal policies and procedures, ensuring compliance with regulatory and accreditation standards.
• Engage, inform, and assist members in making decisions aimed at achieving and sustaining optimal health.
• Conduct consultations through telehealth or virtual platforms.
• CDCES (Certified Diabetes Care and Education Specialist) certification is required.
• Certification as a Diabetic Educator.
• Must be a Registered Dietitian with a valid license.
• A minimum of 3 years of coaching experience is mandatory.
• Bachelor’s degree in a health-related discipline, such as Dietetics, Health Education, Wellness, Health Promotion, or equivalent experience.
• Experience in wellness-related fields, including diabetes management, weight control, stress management, nutrition, physical activity, or behavioral changes for a healthy lifestyle.
• Strong ability to communicate, solve problems, and collaborate effectively with individuals.
• Good judgment in complex situations is essential.
• Health Coach Certification or Fitness certification is preferred.
• Experience with motivational interviewing, active listening, the Transtheoretical Model, mindfulness techniques, and other evidence-based coaching methods is advantageous.
• Proficient in computer skills, including Microsoft Office applications such as Excel, Word, PowerPoint, and Outlook.
• Capacity to cultivate relationships with wellness champions and foster a health-oriented culture.
• Availability to work Monday through Friday, including one late night until 9 p.m. EST each week.
