This is a fully remote position, open to applicants in California.
• Direct the operational implementation of innovative care models for Medicare Advantage patients with chronic kidney disease.
• Ensure the achievement of quality and performance benchmarks, including STARs metrics for managing hypertension and diabetes, medication compliance, nephrologist consultations, and optimal initiation of treatment.
• Lead a team of Integrated Kidney Care (IKC) RNs responsible for case management of dialysis and CKD patients.
• Supervise two team leaders and provide support to non-clinical IKC care coordinators.
• Assist in the recruitment of RNs and manage the onboarding process for new nursing staff.
• Offer continuous clinical guidance and training for RNs.
• Act as a pivotal leader in clinical and operational strategy alongside senior and clinical/operations leadership.
• Oversee Disease Management programs within the assigned market or region.
• Analyze and review data to enhance patient care outcomes.
• Develop and maintain strong relationships with both internal and external stakeholders.
• Support Regional Operations Supervisors in reporting efforts.
• Provide leadership and support to direct reports.
• Manage employment-related activities for teammates, including payroll, paid time off, hiring, onboarding, professional development, performance evaluations, and disciplinary procedures.
• Ensure adherence to relevant regulations and DaVita policies.
• Monitor compliance for risk management and quality assurance.
• Meet quality and metric objectives in Integrated Kidney Care programs.
• Coordinate activities with the DaVita IKC management team, Regional Medical Directors, cross-functional departments, Registered Nurses, and other key personnel.
• Assess processes for cost efficiency and reduction of operational expenses.
• Execute initiatives and evaluate their effects on expected outcomes.
• Create and submit reports and analyses regarding team performance.
• Travel within the market and occasionally outside of it as required.
• A minimum of 5+ years of management experience in leading and supervising teams is required.
• Proven track record of managing data-driven outcomes and process improvements is necessary.
• Experience in primary care and value-based care is essential.
• Strong communication skills regarding medical management, operational, and business-related matters are required.
• Current CPR certification is mandatory.
• Proficient in MS Word, Excel, PowerPoint, and Outlook with intermediate computer skills is required.
• A home office setup with internet connectivity of at least 1MB upload and 1MB download speed is required.
• Must be willing and able to lead remotely from a home office.
• Ability to travel as necessary, including within the market and occasionally outside of it.
• Must reside in MDT/PDT/CDT time zones.
