This is a fully remote position, open to applicants in North Carolina.
• Oversee and engage with members who are receiving Tailored Care Management, providing necessary support.
• Conduct thorough assessments to identify members’ needs and evaluate outcomes to assess the suitability and efficacy of care provided.
• Ensure the development and ongoing management of integrated care plans that address a member’s physical health, mental health, social support, lifestyle, community participation, and potential risks to well-being, among other factors.
• Organize regular meetings with members through both in-person and telephonic methods.
• Collaborate with healthcare providers to collect and disseminate information pertinent to each member’s care, including comprehensive assessments and follow-up evaluations.
• Must fulfill the criteria for Qualified Professional (QP) status.
• Fully licensed by the North Carolina governing board that oversees Human Services professionals; OR a fully licensed RN authorized to practice in North Carolina by the NC Board of Nursing, possessing four (4) years of full-time experience with the MH/DD/SA population served.
• Must hold a master’s degree in a human service field from an accredited college or university, with one year of full-time experience (pre or post-graduate) accumulated in the MH/DD/SA field, or be a substance abuse professional with one year of full-time supervised experience in alcoholism and drug abuse counseling; OR
• A bachelor’s degree in a human service field from an accredited college or university, with two years of full-time experience (pre or post-bachelor’s degree) in the MH/DD/SA field, or a substance abuse professional with two years of full-time supervised experience in alcoholism and drug abuse counseling; OR
• A bachelor’s degree in a field other than human services from an accredited college or university, with four years of full-time experience (pre or post-bachelor’s degree) in the MH/DD/SA field, or a substance abuse professional with four years of full-time supervised experience in alcoholism and drug abuse counseling.
• Must possess a valid driver’s license.
• Must reside within Trillium’s Southern Region, which encompasses the following counties: Carteret, Craven, Duplin, Jones, Lenoir, Onslow, Pender, Sampson, and Wayne.
• Must be willing to travel within the designated catchment area as needed.
