This is a fully remote position, open to applicants in Alabama, +7 more states.
• Conduct remote monitoring and principal care management for patients under the guidance of the medical team.
• Continuously track and oversee patient data communication among medical team members, engaging with patients and their families to provide clinical summaries.
• Support clinicians in collecting medical information for patients between visits by utilizing various electronic health record systems.
• Monthly reporting of any medical issues for patients, including falls, medication concerns, behavioral issues, and recent hospitalizations.
• Complete PCM tasks as assigned, ensuring that all interventions are addressed each month for approximately 15 patients daily.
• Directly report any issues to the ancillary program manager and/or Chief Clinical Officer as necessary.
• Assist with additional employee training as required and provide support to new employees needing extra assistance through “shadowing.”
• A minimum of a High School Diploma is required.
• Strong organizational skills and a proven ability to follow up effectively.
• Basic knowledge of medical terminology, attention to detail, and exceptional written and verbal communication skills are essential.
• Comprehensive understanding of healthcare technologies, including but not limited to: Electronic Health Record Systems, Microsoft Outlook, Microsoft Excel, and internet/cloud-based applications.
• A quiet, distraction-free workspace with internet access at home is required for remote work.
