Prescribing Clinical Pharmacist – UK Remote

This is a fully remote position, open to applicants in United Kingdom.

📋 Description

• Deliver specialized pharmaceutical assistance to patients receiving support through NHS Partners

• Conduct reviews for patients with chronic conditions within the scope of clinical competencies

• Gather and document patients’ health information in a comprehensive manner

• Analyze, risk stratify, and manage health information in accordance with standard operating procedures and NICE guidelines

• Offer evidence-based lifestyle recommendations

• Prescribe or modify medications for patients with chronic conditions

• Uphold patient safety and dignity at all times

• Establish and nurture trust with patients

• Keep up-to-date clinical notes within Suvera’s and GP Partners systems

• Actively participate in the Quality Outcomes Framework (QOF)

• Perform additional administrative tasks as reasonably required

⛳️ Requirements

• At least 12 months of experience as a clinical pharmacist in an NHS Primary Care environment

• Current membership in a medical professional defense organization

• Independent prescriber with completion of a GPhC-accredited course or equivalent

• Currently registered with the GPC and compliant with the code of Professional Conduct

• Outstanding communication abilities, both verbal and written

• Comfortable communicating with patients through email, messaging, phone, or video calls

• Proficient in using software systems such as Emis Web, SystmOne, and Microsoft Office

• Competent in managing conditions including Hypertension, Type 2 Diabetes, Asthma, Lipids, Structured Medication Reviews, Frailty, CKD, heart failure, and COPD
