This is a fully remote position, open to applicants in Oregon.
• Act as the clinical escalation authority throughout the MSK care process, directing triage recommendations and addressing complex or ambiguous clinical scenarios.
• Perform evidence-based clinical intake evaluations and identify suitable care pathways.
• Offer clinical advice, education, and support for shared decision-making.
• Establish trustworthy relationships with patients through comprehensive telephonic engagement.
• Inform patients about Motion MSK programs and available care resources.
• Conduct outreach and proactive follow-up to initiate or modify care pathways.
• Monitor patient progress and re-evaluate care plans when necessary.
• Utilize motivational interviewing techniques to encourage engagement, adherence, and persistence.
• Coordinate care among patients, caregivers, primary care providers, specialists, and affiliate providers.
• Supervise navigation pathways for patients transitioning to surgical care.
• Collaborate with Care Coordinators, Outreach Specialists, and cross-functional teams.
• Handle high-volume inbound and outbound phone communications.
• Document patient interactions and care navigation activities within CRM and EMR systems.
• Manage organized workflows that support care coordination, clinical oversight, and patient experience.
• Execute other related duties that support patient care needs, workflows, and business objectives.
• Doctor of Physical Therapy (DPT), Doctor of Physiotherapy, or Master of Physical Therapy (MPT).
• Active, unrestricted Physical Therapist license in Oregon.
• Capability to acquire and maintain licensure in additional states and/or PT Compact privileges as required.
• Over 5 years of clinical experience in orthopedics or musculoskeletal care.
• Strong clinical judgment and triage capabilities.
• Experience with motivational interviewing, patient coaching, and fostering behavior change.
• Comfortable working in a predominantly telephonic environment with incoming and outgoing patient communications.
• Exceptional interpersonal and communication skills.
• Experience collaborating effectively within multidisciplinary care teams.
• Ability to operate within structured clinical workflows and guidelines.
• Familiarity with EMR systems and remote clinical or care navigation tools.
• Access to a private, secure, HIPAA-compliant remote work setting.
• Must reside in the Pacific, Mountain, or Central time zones.
• Willingness to provide coverage across additional U.S. time zones as necessary.
